Bayview Center offers case management services designed to support the recovery of individuals with severe and persistent mental illness. Case Managers come to your home and work with you to develop a plan for helping you to live better and achieve your goals. Services include an initial assessment in order to recommend the services and treatment that will benefit you. Your Case Manager will help you connect to resources in the community and will assist you in setting up needed appointments, arranging transportation if necessary and will follow up to make sure you are getting the care you need.
